The Marymount University medical forms must be completed and submitted to the Student Health Services before the deadline of August 1, 2024. If noncompliant, class attendance may be restricted.

All Marymount students born after December 31, 1956, must complete an immunization history. This documentation provides proof of compliance with immunization requirements mandated by the Commonwealth of Virginia and Marymount University. This requirement is separate and in addition to any department specific requirements. The required forms can be found at emailed to shealthc@marymount.edu. Records must be in English to ensure accuracy.

Exemptions:

Pursuant to Virginia law, no student shall be admitted by a school unless the student submits documentary proof of immunization to the school or unless the student is exempted from immunization as outlined in the Code of Virginia.  The following are exceptions to VA immunization requirements: (i) the student submits an affidavit stating that the administration of immunizing agents conflicts with the student’s religious tenets or practices; or (ii) the student has written certification from a licensed physician, a licensed advanced practice registered nurse, or a local health department that one or more of the required immunizations may be detrimental to the student’s health, indicating the specific nature and probable duration of the medical condition or circumstance that contraindicates immunization.

Every Marymount University student must meet the VA state immunization requirements to attend the University.   Students must provide the University’s Student Health Services documentation that they have met VA immunization requirements unless the student is exempted from immunization as permitted by law.  These procedures outline the process for students to request an exception from immunization under Virginia law.

Religious exemption:

If the administration of immunizing agents is against your religious tenets or practices, you may complete the religious exemption.  Signing this affidavit, you understand, that in the case of an outbreak, potential epidemic, or epidemic of a vaccine-preventable disease at Marymount University, the State or County Commissioner may require you to be excluded from the university and associated events for your own protection until the risk has passed.

Medical exemption:

If the administration of immunization(s) is detrimental to the student’s health, the student can be exempt from the university vaccination requirements. You may complete the medical exemption. In the case of an outbreak, potential epidemic, or epidemic of a vaccine-preventable disease at Marymount University, the State or County Commissioner may require you to be excluded from the university and associated events for your own protection until the risk has passed.

Failure to comply with VA immunization law, you may be prohibited from attending classes.
