Key replacement form

Please fill this form out to request a new room key.  The price for a new key is $350.00 that will be charged to your eBill.  Please be advised that requesting a new key may result in a re-core, which will mean your roommate will be getting a new key as well.  Please allow up to 7 Business days to receive the key as we have to contact the 3rd party company to assist.
