SAS Student Intake Forms

To register with SAS as a student with a disability, complete these two forms:

1. SAS Student Registration Form

REQUIRED FOR ALL NEW ACCOMMODATION REQUESTS

Students requesting approval for accommodations at Marymount must submit a completed “SAS Registration Form” to Student Access Services (SAS) directly, along with the “SAS Information Release Form” (below) and their disability documentation.  The requesting student is responsible for scheduling an appointment to meet with SAS staff.  The appointment can be scheduled through Starfish, through email to access@marymount.edu or by contacting the Student Academic Hub.

  • This form is reviewed before or during the student’s SAS intake meeting. It is used in conjunction with the student’s disability documentation (and any other required SAS forms) to identify the most appropriate accommodations for the student at MU.

2. SAS Student Information Release Form

REQUIRED FOR ALL NEW ACCOMMODATION REQUESTS

Students must submit this completed “SAS Information Release Form” with their “SAS Registration Form” and disability documentation to the SAS office directly. The requesting student is responsible for scheduling an appointmnet to meet with SAS staff.  The appointment can be scheduled through Starfish, through email to access@marymount.edu or by contacting the Student Academic Hub.

  • This form indicates who (if anyone) SAS is allowed to speak with or respond to (i.e., family members, advocates/support people, medical providers, etc.) should they have questions about student’s disability status or SAS-approved accommodations. SAS cannot release information or discuss the student’s confidential SAS records with anyone who the student has not listed on this form.

Testing Request Form

 

E-Text/Alternative Text Format Request Form

Students who are approved for the use of alternative textbook formats (i.e., digital, audio) may request them through SAS, after they have purchased the textbook hardcopy on their own. Students should submit an individual form for each textbook and scan an attachment to access@marymount.edu. It may take up to four weeks to receive the alternative format, so students should submit the requests as early as possible. Additional instructions are described on the form.

Sign Language Interpreter Services – Student Request Form

Students who require a sign language interpreter for an MU class or MU-sponsored event must submit this completed request form to SAS, at least 2-3 weeks prior to the date that an interpreter will be needed: access@marymount.edu, or 703-284-1538.

Personal Care Attendant Agreement Request Form

Students who require personal care attendant services who wish to have the same independent experience as all other college students are encouraged to hire an impartial personal care attendant (PCA), who is not a family member or close friend. Any student requiring the use of a PCA must submit this form in order to register the PCA as a member of the Marymount community.

Faculty members sign after meeting with students who have approved accommodations each semester.

Testing Slip (Fillable PDF)

Sign Language Interpreter Services – Faculty/Staff Event Request Form (for faculty/staff events)

Professors, staff and invited guests who require a sign language interpreter or captioning services must submit this completed request form to SAS, at least 2-3 weeks prior to the date that services will be needed. Contact SAS with any questions/concerns: access@marymount.edu, or 703-284-1538.
