Psychological Disorders (e.g., anxiety, depression, PTSD):<\/h3>\n\n- Comprehensive report from a psychologist, psychiatrist, or licensed mental health professional.<\/li>\n
- Must include diagnosis, functional limitations, treatment\/medications, supporting clinical data, and recommendations.<\/li>\n<\/ul>\n
Traumatic Brain Injury:<\/h3>\n\n- Evaluation from a rehabilitation counselor, neuropsychologist, or relevant specialist.<\/li>\n
- Assessment of cognitive abilities (memory, processing speed), academic impact, and treatment history.<\/li>\n<\/ul>\n
Learning Disabilities:<\/h3>\n\n- Psychoeducational\/Neuropsychological evaluation from a psychologist, school psychologist, or diagnostician.<\/li>\n
- Reports should generally be within 5 years.<\/li>\n

\n- Diagnostic interview and educational history.<\/li>\n
- Standardized test results with standard scores (e.g., WAIS, Woodcock-Johnson).<\/li>\n

- Recommendations with rationale.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n
ADHD\/ADD:<\/h3>\n\n- Psychoeducational\/Neuropsychological evaluation from a psychologist, school psychologist, or diagnostician OR a letter by a physician, psychologist, or neurologist.<\/li>\n
- Reports should generally be within 5 years.<\/li>\n
- Must include:\n
\n- Diagnostic interview and history of symptoms across settings.<\/li>\n
- Standardized measures or rating scales.<\/li>\n
- DSM-5 diagnosis (medication history alone is not sufficient).<\/li>\n
- Functional limitations and recommended accommodations.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n
Emotional Support Animals (ESAs):<\/h3>\n\n- Documentation must come from a licensed clinician with whom the student has an ongoing therapeutic relationship.\n
\n- Documentation must specify how long the provider has been treating the student and the frequency of the appointments.<\/li>\n<\/ul>\n<\/li>\n
- Must be from the student\u2019s state of residence or the District of Columbia, Maryland or Virginia.<\/li>\n
- ESA must be specifically prescribed as part of a treatment plan.<\/li>\n
- Letters purchased online or from paid ESA services will not be accepted.<\/li>\n<\/ul>\n
