The U.S. Department of Homeland Security requires all F-1 students on OPT to provide the following employer information. We ask that you complete this form below for us, so that we can enter this information into SEVIS and issue you a new I-20 with your OPT employment listed. Your record will automatically be terminated if you do not have employment listed for 90 days. You are only allowed 90 days of unemployment. Once you complete this form and submit a copy of your EAD card, we will then issue you a new I-20 with your OPT employment information.
